Community Flu Shot Clinic

Your Care is coming to Crooked River Ranch Fire & Rescue to make it convenient for you to protect you and the ones you care about against the flu!

When: Thursday, October 7th

Time: 2:00 pm to 6:00 pm

Where: Crooked River Ranch Fire Station

Who is this for?: Crooked River Ranch residents 2 years or older who wishes to get a flu vaccination.

Social Distancing & What You Will Need:

We will do all we can to move everyone through quickly and maintain social distance. A mask will be required to be worn when coming in for your flu vaccination. You can also pre-fill out the flu vaccination questionnaire below!

Influenza Vaccination Questionnaire

Flu shots are covered by most insurance plans, including Medicare Part B, however, patients should contact their insurance carrier prior to attending one of the clinics for verification. For individuals not covered by medical insurance, the cost for the 4-Way Vaccine (ages 2 to 64 years) is $30.00 and the high dose vaccine (65 years and older) is $60.00.

Please bring a photocopy of your ID and insurance card!
